Photocopy or scanned copy of ONE (1) of your VALID IDENTIFICATION (ID) CARDS.

4. PROOF OF INCOME/ SOURCE OF FUND
Photocopy or scanned copy of proof of income/source of fund (as applicable).

5. PHILIPPINE PASSPORT (FOR FORMER NATURAL-BORN FILIPINO)
Photocopy or scanned copy of your Philippine Passport (as applicable).

6. CERTIFICATE OF REACQUISITION/RETENTION OF PHILIPPINE CITIZENSHIP
Photocopy or scanned copy of your Certificate of Reacquisition/Retention of Philippine Citizenship (as applicable).
